For physicians

Refer a patient for an oral appliance

You diagnose and you decide. When an oral appliance fits your patient's diagnosis, we make it and we tell you how it's going.

We are two general dentists. We don't diagnose sleep apnea, order sleep tests or change a treatment plan. We make and fit the appliance, check your patient's teeth, jaw and bite, and write to you after each step.

Where the line is

  • We don't diagnose sleep apnea

  • We don't order or read sleep tests

  • We write to you after the fitting and after each follow-up

  • A repeat sleep test stays your decision

What we ask you to send

Call us first and we'll tell you how to send it. Please don't use the website's contact form for patient details.

  1. 01

    The recommendation

Your recommendation for an oral appliance, with anything you want us to know about your patient's other treatment.

  2. 02

    The sleep test report

The report your diagnosis rests on, so we can see what you saw.

  3. 03

    Your preferences

How you want to hear from us, and whether you want a repeat sleep test timed to the follow-up.

Services

What we send back

  • After the screening

What we saw in your patient's mouth, jaw and bite, and what we told them.

  • After the fitting

That the appliance has been fitted, how it was set and when the follow-up is.

  • After each follow-up

Fit changes, any change in teeth or bite, and what the patient told us.

  • On request

Copies of our notes for your patient's chart.

Questions from physicians

Will you change my patient's treatment plan?

No. Treatment decisions, including whether to repeat a sleep test or try another therapy, are yours and your patient's.

What if my patient can't tolerate the appliance?

We tell you, with what we tried. You decide the next step.

Do you see patients who haven't had a sleep test?

For a screening visit, yes. We don't make an appliance without a diagnosis, and we'll write to you with your patient's permission.

How do you handle patients' records?

We keep records private and share them only as the law allows.

Refer a patient

Call the office and we'll tell you how to send a referral. We answer during office hours and return calls the same day when we can.
